Complete this form to apply for or renew a private hire operator licence. Before you start this form, you will need to have digital copies of the following to upload them to your application: Company door stickers Rear Company stickers (if you use these on your vehicles) Tariff card Public liability insurance certificate (if you have a public waiting room) Basic DBS certificates for all sole traders/partners/directors/owners who do not hold a Preston City Council Taxi Driver badge On completing the application please have your payment card ready and we will take you to our secure payment site to pay the fee. The rules are changing if you're an individual, company or any type of partnership applying for a licence as a: taxi driver private hire driver private hire vehicle operator If you make an application on or after 4 April 2022 you'll need to complete a tax check with HM Revenue and Customs (HMRC) if you're: renewing a licence applying for the same type of licence you previously held, that ceased being valid less than a year ago applying for the same type of licence you already hold with another licensing authority You must carry out the tax check yourself. You cannot ask a tax agent or adviser to do this on your behalf. If a tax check is not completed, the licensing authority will be unable to consider your application to renew your licence. Each applicant will be asked to provide their individual HMRC Tax Check Code on the next page of this application form. Public Liability Insurance If you have a public waiting room you will need Public Liablity Insurance. You must provide a copy of your public liability insurance certificate by uploading it below.
