1. This form is to be used to be considered for a Council Tax discount if you receive Child Benefit.
  
  For further information and proof you will be required to supply please visit child benefit form page.
  
  This form has one page of questions and should take around 5 minutes to complete.
2. The Applicant (this form must be completed by the person who is liable to pay the Council Tax)

  1. The dwelling must be his/her sole main residence
  2. Name of person

  4. Date Child Benefit Ceases (optional)
  5. DSS allowance number (optional)
  6. How many residents of the property will be aged 18 or over by next April?
  7. The discount will cease on the day entitlement to Child Benefit ceases. You must notify us if this occurs prior to the date given above.
