Application for a Change of Details - Driver Licence (Hackney and Private Hire)
1. Please complete this form if you wish to change your name, your address or the operator you are working for (private hire only) in relation to your driver badge license.
  
  If you have changed your name or your address you must have your updated driving licence photocard showing your new details, as you will need to upload a copy of it to the form.
  
  If you also have a vehicle licence with us and have changed your name or your address, you must also have your updated V5 document showing your new details, as you will need to upload a copy of it to the form.
  
  This form is one page long and may take approximately 5 minutes to complete.
2. PLEASE NOTE: You need to ensure you have updated your details with Taxi Plus if you are regsitered with them.
  
  You also need to ensure your details are updated with the DBS Online Update Service.
  
  Failure to update your registered details with either of the above services can result in your driver licence being suspended due to you no longer being correctly registered for DBS automatic checks and updates.
